MENDOTA WARRANTY QUALIFICATION & SERVICE REFERENCE FORM
As a part of Mendota's on-going program of customer satisfaction, this Form is used to verify proper installation
and operation. It is important as a reference for future service, it insures long life and trouble-free operation of the
Mendota product, and it qual
ifies the owner for Mendota’s warranty. The owner should sign this Form when it is
completed, and mail a copy, along with the Warranty Registration, to Mendota.

MODIFY OR REMOVE THE DAMPER TO ATTAIN THE MINIMUM OPENING (see page 7).

CONNECT THE APPLIANCE TO THE GAS LINE & CHECK FOR GAS LEAKS (see page 7).

CHECK FOR THE CORRECT GAS PRESSURE AT THE MANIFOLD (see page 7).


