and follow the instructions to set up an account.Using the MyHealthPoint

Portal at www.myhealthpoint.com patients can wirelessly transmit and

evaluate their daily readings on their PC. Stored results are wirelessly

transmitted from the meter to the Portal for posting.

The BLE Smart meter uploads readings on MYHEALTHPOINT

Patient

portal by transmitting them via a paired Bluetooth mobile device. The user

can see the most recent uploaded reading on the app screen once the

results are transmitted successfully.

STEP 1 — NEW MEMBER REGISTRATION on MYHEALTHPOINT

When you set up your membership account on the myhealthpoint.com

website, your meter will automatically transmit your blood glucose test

results via the bluetooth connection.

STEP 2 — LOG IN to MYHEALTHPOINT

using the log in and password

you created during New Member Registration.

WARRANTY:

Entra Health warrants to the original purchaser that this device will be

free from defects in workmanship for three years from the date of original

purchase.

Limitations of Warranty

This warranty is subject to the following exceptions and limitations. Entra

Health shall not be required to replace any units which are damaged

or malfunction due to abuse, accidents, alteration, neglect, misuse,

maintenance by someone other than Entra Health, or failure to operate in

accordance with the instructions.
