6476B BROADBAND BOOSTER  

 

 

 

MATERIAL RETURN PROCEDURE 

 

 

In order to efficiently handle equipment or components returned for 

repair or sent out on loan, Axcera requests that each returned item be 

accompanied by a Return Material Authorization Number (RMA#). 

 

To obtain an RMA follow the procedures below: 

 

 

Call Axcera Customer Service at 1-724-873-8100  

 

or FAX 1-724-873-8105 

 

 

A Service Engineer will provide you with an RMA# 

 

 

Write the RMA# on the packing list or in the case of 

repairs, a note describing the reason for return. Also, be  

sure to include contact information. 

 

 

Send ALL RMA items to the following address
