It is highly recommended that a Nitrox fill log be maintained when filling Scuba cylinders to document
the following information. This log must be of permanent binding style with no loose pages.

•
Supplier’s check of oxygen content (%O

) plus signature and date

User’s check of oxygen content (%O

) plus signature and date
•
Fill Pressure
•
MOD (Maximum Operating Depth) in user’s handwriting

Nitrox certifying agency and card number
