RESPONSES TO QUESTIONNAIRE

Do you have epilepsy or have you ever had a convulsion or a seizure?

Have you ever had a fainting spell?
If the respondent answers yes or don’t know, do not administer TMS.

Have you ever had severe (i.e. followed by loss of consciousness) head trauma?
If the respondent answers yes or don’t know, do not administer TMS.

Do you have any hearing problems or ringing in your ears? (Advise earplugs
must be worn as possible risk of temporary tinnitus)
If the respondent answers yes or don’t know, do not administer TMS .

Are you pregnant or is there any chance that you might be?
If the respondent answers yes or don’t know, do not administer TMS.

Do you have metal in the brain/skull (except titanium)? (e.g. splinters,
fragments, clips etc.)
If the respondent answers yes or don’t know, do not administer TMS.

Have you had surgery in the last 12 months above the neck?
If the respondent answers yes or don’t know, do not administer TMS.

Do you have a cochlear implant?
If the respondent answers yes or don’t know, do not administer TMS.

Do you have an implanted neurostimulator? (e.g. DBS, epidural, subdural, VNS)
If the respondent answers yes or don’t know, do not administer TMS.

Do you have any heart problems, a cardiac pacemaker, intracardiac lines or metal
in your body?
If the respondent answers yes or don’t know, do not administer TMS.

Do you have a medication infusion device?
If the respondent answers yes or don’t know, do not administer T MS.

Are you taking any medications such as anti-depressants, psychoactive,
neurological, psychiatric or recreational drugs?
If the respondent answers yes or don’t know, do not administer TMS.

Have you consumed alcohol within the last 12 hours?
Please use the following guidelines for alcohol consumption. If the daily or weekly
amounts have been exceeded do not administer TMS.
