Operating Procedure during Accident or 
Breakdown 

During operation, should an accident or breakdown occur, users shall perform 
following: 

1.

 

Operator  shall  stop  the  machine  operation  immediately  by  toggling  the 

[Cycle  Start]

  button  at  the  control  panel  to  stop  the  process.  Pressing 

EMO button if necessary. Inform others user of the status of the machine. 
Alert and report to supervisor and maintenance personnel. Monitor status 
of the machine while awaiting assistance. 

2.

 

Maintenance shall access the safety in the area of the machine. Approach 
the machine; look for the alarm message prompt on touch screen. Monitor 
status  of  the  machine.  Power  off  machine.  Perform  lock  out  tag  out. 
Visual  inspect  overall  of  the  machine,  perform  troubleshooting  which 
relates  to  the  problems,  accident  or  breakdown.  Alert  engineers  for 
assistance  and  advice.  Determine  the  possible  root  cause  of  the  fault. 
Make a record of the incident. 

3.

 

Engineer  shall  engage  maintenance  and  operator  to  determine  the  root 
cause  of  the  fault,  accident  or  breakdown.  Review  the  records  and 
perform corrective action.  Power up  the machine again and monitor the 
machine performance. Ensure that the machine operation is stable before 
allowing operator to operate it.
