License reference code from the Administrator Menu -> Licenses screen.
Ethernet MAC address from the Administrator Menu -> Licenses screen.
Serial number from rear of Net-Log.
Number of consecutive streams to be licensed (2, 5 or upgrade 2-5)
Your Name and Address
Agent or Distributor purchased from
At least one of the following:
Telephone (inc country code)
Fax number (inc country code)
Email address
