The Pre-Installation Evaluation must be completed prior to the arrival of the SkyVision integration 

system installation team. This form can be used to describe a multi-room installation if the  
details that are room specific can be noted on the final page.  In the event this evaluation is  
not completed properly and additional visits are required, the customer will be billed at  
$1900.00 per day, per technician.
