The Return Declaration (page 57) and the Declaration of Decontamination (page 
59) serve for maintaining the safety and health of our employees. 
 
Fill out the forms and attach them when returning centrifuges, accessories, and 
spare parts. Please understand that we cannot carry out any work before we 
have the declarations.
