Before returning any product to JDSU, you
must first request a Return Merchandise
Authorization Number by contacting our
Customer Service Dept. at (0) -00.
No shipments will be accepted without
this number, which must be clearly
marked on the shipping label.
Ship the equipment with a copy of the
sales receipt, if available.
Attach a description of the operational
problem.
Include a contact name, phone number
and E-mail address.
Pack securely to prevent damage
during shipping.
