Follow these steps to initiate repair of your instrument:

1. Repair Form

• For a single instrument, find online at

• Complete the form by providing all information requested, such as:

2. Package Your Return Carefully

• Use the original shipping carton and cushions if available
• Include all components
• Include the repair form
