5.9 System Start-Up Report
A copy of the system “Start-Up Report” can be found on the following pages. This report needs to
be filled out thoroughly by a qualified service technician and returned to Desert Aire for warranty
validation. Please ensure that the model and serial number of the unit are noted on this form.
The model and serial number can be found on the system’s rating plate located on or near the
electrical compartment service door. Failure to complete and return this form will void the unit’s
warranty. These reports are also helpful when trying to correct existing problems. Should you
need system diagnosis help, fax the completed worksheet to Desert Aire’s Service Department
using the number provided. Be sure to include your name and a telephone number when you can
be reached.
