one 4-1/2” X 8” PLATE (57), two 1/2” WASHERS (84), two 3/8” WASHERS (80), one 1/2” LOCK NUT (85), and two 3/8” LOCK NUTS (82).

(NOTE:

Make sure 3/8” countersunk bolts are facing down.)
