Specify whether the patient is currently using optical correction.

Set the category of the patient if part of a study group.
5. [Mode]
Initial assessment of eye deviation. If there is no initial assessment, do not press on any of the buttons.

Any additional notes or comments that you wish to enter for the patient.

ESTS TO RUN
This section of the screen allows you to identify which tests will run in the near and far procedures.
1. Test selection
This tab indicates that you are selecting which tests to run.
Press on each test that you wish to run. The button will change to green to indicate that the test has
been selected: .
2. [Near / Far]
Based on the selected toggle, the tests will be run in near or far mode. This toggle will also change the
available tests view.
