under which the system is sold, health related contaminants claimed to be reduced and the date of
initial registration.
Please let me know if there are any errors.
You are required to notify the department if you plan to make any changes in the performance data
sheet, in the design of the water treatment device or in the trade names under which the model is sold.
If you are going to retest the product in order to add claims to remove more health related
contaminants, a new protocol and laboratory approval will be required. You are also asked to notify the
department if there is a change in the business name, address or contact person.
