SW5 Card Index Selection

Figure 2-1: SW5 Switch

The SW5 switch is used to set the card index. For example, if the
switch is set to 1-0-0-0 (ON-OFF-OFF-OFF), card ID is 1. The
index value can be from 0 to 15. Details are as follows.
