IMPORTANT:
Should repair be needed within the warranty period, please call
Customer Care at 1 (888) EarChek • 1 (888) 327-2435. To ensure prompt
response and service, do not return your EarCheck without first obtaining a
Return Authorization number from Innovia Medical. Call and explain the exact
nature of the problem. Please indicate the lot number and serial number of the
instrument located in the battery compartment. Be sure to include your phone
number on any correspondence. Return the instrument to Innovia Medical
insured and postage prepaid.

Use your EarCheck Middle Ear Monitor for three weeks until you are
fully familiar with it. For assistance visit our website at
www.earcheck.com or call 1-888-EarChek (327-2435). After that time, if
you are not completely satisfied with your Earcheck, we will refund your
purchase price.
Requirements for Refund
You must call Customer Care at 1-888-EarChek (327-2435) within 30
days of purchase, for a Return Authorization number and shipping
address. Send your EarCheck prepaid and insured, along with your
name, address, phone, email
and Return Authorization. Include the
original sales receipt
indicating the date of purchase.
Sorry: No refunds will be made if damaged by accident, neglect or
unreasonable use.
The EarCheck must be shipped in its original packaging or securely
packed to avoid damage during shipping and be postmarked no later
than 30 days after the date of purchase.
