The following symptoms could indicate a serious problem with your wheelchair. If necessary, contact your Quan-
tum Rehab Provider. When calling, have the model number, serial number, and nature of the problem.
Uneven wear on any of the tires
Bent or broken wheel assemblies
Loose or missing hardware that attaches the transit securement brackets or positioning belt anchor to the
wheelchair
Wheel locks not working properly
