Service checklist
To maximize the lifetime of your wheelchair, we recommend you to get your wheelchair checked at least once
a year, by an authorised Van Os Medical dealer. The form below can be used for this check. The intensity of the
wheelchairs use can vary greatly from person to person. Therefore, it may be that your wheelchair needs a check
several times a year. Furthermore, maintenance is not covered under warranty.
