Service checklist
To maximize the litefime of your wheelchair, we recommend you to get your wheelchair checked,at least once
a year, by an authorized Van Os Medical dealer. The form below can be used for this check. The intensity of the
wheelchair users can vary greatly from person to person. Therefore, it may be that your wheelchair needs a check
several times a year. Furthermore, maintenance is not covered under warranty.
