Customer Service Checklist

When speaking with the Customer Service representative:

1. Have your

2. Locate the model number (A) and

serial number (B) on the back of

the meter.

3. Locate the test strips’ expiration

date on the bottle.

4. Check the battery status.

that may not be included in your kit are indicated by an asterisk (*).

INTENDED USE

The

ontour

GEN blood glucose monitoring system is intended

to be used for the quantitative measurement of glucose in fresh capillary

whole blood drawn from the fingertips. The

ontour

ext

GEN blood

glucose monitoring system is intended to be used by a single person

and should not be shared. The

ontour

ext

GEN blood glucose

monitoring system is intended for self-testing outside the body (in vitro

diagnostic use) by people with diabetes at home as an aid in monitoring

the effectiveness of a diabetes control program.

The

GEN blood glucose monitoring system should

not be used for the diagnosis of or screening for diabetes or for

neonatal use. The

ontour

ext

test strips are for use with the

ontour

ext

GEN blood glucose meter to quantitatively measure

glucose in fresh capillary whole blood drawn from the fingertips.

The system is intended for in vitro diagnostic use only.
