1. Disconnect VitalPatch in VistaPoint
When monitoring is complete, select
Disconnect
VitalPatch
from the menu. Select one of the following
options:
• Disconnect VitalPatch:
Choose this option if you will
be applying a new VitalPatch.
• Discharge Patient:
Choose this option if you will
NOT be applying a new VitalPatch. All devices will be
disconnected. Use as directed by your care team.
2. Remove VitalPatch

If a new VitalPatch will not be applied, turn off the
VistaTablet or VistaPhone by pressing and holding the
Power Key
on until the Options menu displays, and then
select
Power Off
.
CALL 911 if you are experiencing an emergency.
If your care team asks you to report symptoms that you
experience while wearing the VitalPatch, do the following
when you feel a symptom:
1.
When it is safe to do so, unlock the VistaTablet or
VistaPhone and tap the
Report Symptom
button.
2.
From the list of options, tap symptom(s) to select. Tap
Other
if the symptom is not listed. Tap
Next
.
Note:
The list of symptoms will vary depending on the
condition being monitored by your care team.
3.
If you selected
Other
as a symptom, you will be
asked to enter a description of the symptom(s). Enter
the information and tap
Next
.
4.
Select the activity level (
Low
,
Medium
or
High
) that
you experienced when the symptom occurred. Tap
Send Symptom
when complete.

Discharge or Disconnect
Choose one of the following discharge/disconnect options:
Disconnect VitalPatch and
continue monitoring patient.
Discharge patient from the
monitoring system.
Cancel
Tap “Disconnect
Patch”
Choose a Disconnect Option
