Wanneer er op de toets “Braden Scale” (Braden-
schaal) op het scherm

i

Bed screen wordt gedrukt,

wordt figuur 10 .22a weergegeven .

De Braden-schaal is een vragenlijst met 6 vragen
die de zorgverlener kan gebruiken om de kans te
evalueren dat een patiënt doorligwonden krijgt .

Wanneer er op de

voor een van de items 1-4

wordt gedrukt, wordt extra informatie over dat item
gegeven . Zie voorbeeld voor item 2 (Very Limited
[Zeer beperkt]) in figuur 10 .22b .

De vragenlijst levert een getal (score) op dat wordt
gebruikt om te bepalen welke zorg nodig is voor de
huid van de patiënt (zie figuur 10 .22c) . Hoe lager de
score, hoe meer aandacht de zorgverlener dient te
besteden aan de huid van de patiënt .

Wanneer een Braden-schaal volledig is uitgevoerd,
wordt de laatste Braden-score weergegeven op het
hoofdscherm, met de datum van de evaluatie . Het
weergeven van deze score op het hoofdscherm kan
worden in- of uitgeschakeld .
