The orthogonal lead configuration is based on the theory of the heart as centre of a 
three-dimensional system of coordinates: 

lateral axis X

longitudinal axis Y

sagittal axis Z

With the patient lying down, attach the electrodes on a level with the fourth intercostal 
space. With a patient in a seated position, attach the electrodes in the fifth intercostal 
space.

Place all other electrodes in the normal position
