10.12.6 P-SIMV
P-SIMV
– Pressure controlled synchronized intermittent mandatory
ventilation
Description:
In P-SIMV, the respiratory rate, inspiratory
pressure and inspiratory time are set, as
well as the sensitivity criterion for the
occurrence of ventilator triggering by the
patient.
This mode allows the ventilator to apply
predetermined preset cycles in sync with
the patient's inspiratory effort.
Mandatory cycles occur in the pre-
determined window of time (the onset of
inspiration occurs according to the pre-
established
respiratory
rate)
but
synchronized with the patient's trigger.
If there is an apnea, the next cycle will be
triggered by time until the patient's
inspiratory incursions return.
The transition between inspiration and
expiration in the mandatory cycles occurs
after release of the pre-set tidal volume at
given velocity through the flow (or ratio or
inspiratory time). In spontaneous cycles,
cycling (transition between inhalation and
expiration) occurs by flow as it falls to a
certain percentage of its maximum
reached value.
In order to obtain the IMV in this mode,
simply deactivate the support pressure by
setting the support pressure equal to zero
(ΔPS = 0) or the flow and pressure
sensitivities, equal to zero.

(1)
1 - Whenever the set apnea time is reached, the ventilator
triggers a ventilator cycle, the setting of which is based on the
current mode settings.
