Discontinuing Your LATITUDE Cellular Data
Plan

Contact LATITUDE Patient Services to discontinue use
of the LATITUDE Cellular Data Plan. Your subscription
will automatically be discontinued if you miss two or
more payments. You will need to discontinue your
subscription if you stop using the LATITUDE system
or if you want to return to using a landline telephone
connection. Make sure your health care provider is
aware of any changes in service.

For information on returning, replacing, or disposing of
your USB cellular adapter, see page 51.
