Appendix G User Fault Report 

This appendix contains a User Fault Report to be used in the case of a fault with a Vega-MV 
unit. 

The User Fault Report should be filled in by the user if a fault is observed. 

The report aims to:

1.

Record information about a fault.

2.

Help with providing extra information and diagnosis.

3.

Provide key information for speedier fault resolution.
