Return

Procedure

Should

a

malfunction

occur,

contact

Quidel.

If

Quidel

determines

that

the

meter

should

be

returned,

you

will

be

assigned

a

return

authorization

number

and

be

provided

shipping

instructions.

Immediately

after

the

return

has

been

authorized,

Quidel

will

send

out

a

replacement

Quidel

Triage

MeterPro.

Return

the

malfunctioning

meter

to

Quidel.

Note

the

return

authorization

number

on

both

the

shipping

box

and

the

airbill

and

send

the

meter

back

to

Quidel

as

soon

as

possible

following

receipt

of

the

replacement

meter.

Disposal

Procedure

The

product

may

come

into

contact

with

blood

during

testing.

Used

products

therefore

carry

a

risk

of

infection.

When

disposing

of

a

meter

that

has

been

utilized,

please

do

so

in

accordance

to

the

regulations

applicable

in

your

country.

For
